You agree that by completing forms on this Site and clicking the agreement and submit button (in any form), you agree to affix your electronic signature to any submitted form which will have the same effect as physically signing your name. You agree that thereafter you will be accepting documents in electronic form, and will be able to execute them in electronic form.
Hardware and Software Requirements
In order to sign, complete and receive documents electronically you will need the following:
- A personal e-mail address
- A computer or other device with standard e-mail software
- Current version of Chrome, Firefox, Edge or Safari
- A connection to the internet
- A printer if you want to print paper copies
Alternative Methods of Contact
You may choose to contact P50 Digital, LLC by contacting it through a phone call, and inquire about various offered services. You understand that P50 Digital, LLC does not have the ability to submit documents for signatures in non-electronic form, but you may obtain these documents from the underlying service provider itself.
Scope of Consent
Your consent to electronically sign, complete and receive documents relating to your request for information form from P50 Digital, LLC during this session and any future sessions, unless you later withdraw your consent. Additionally, you consent to electronically receive:
- Communications relating to your form request to P50 Digital, LLC;
- Communications from the service providers, including T-Mobile®, or any alternative Service Provider if T-Mobile services are not available in your area;
- Requests for additional information; and
- Notices of actions taken required by law and notices of your rights under applicable laws.
Your consent applies to documents completed, signed or provided via this website, as well as to documents transmitted via email.
Withdrawing Your Consent
If your contact information changes, or you would like to exercise your right to withdraw your consent at any time, you may do so by contacting P50 Digital, LLC. You may update your information or withdraw your consent by calling (in the US only) P50 Digital, LLC, at 1-800-895-2494 or by e-mailing at privacy@p50digital.com.
If you withdraw consent, you will no longer have the ability to sign documents electronically and to receive disclosures and other documents electronically. Your withdrawal of consent is prospective only and will become effective after we have had a reasonable opportunity to act upon it. After withdrawing your consent, please also contact the service provider's representative to make arrangements to receive paper copies of documents and communications.
